Our multidisciplinary team is composed of highly trained professionals, including clinical pharmacists, pharmacy technicians, occupational therapists, care coordinators, mental health advisors, health and wellbeing coaches, social prescribers, mental health nurse specialists, and community wellbeing connectors. We aim to enhance general practice support by offering a tailored range of services to meet the unique needs of our patient population and improve access to appointments
We are committed to quality improvement and addressing health inequalities through various projects designed to enhance patient care. By collaborating closely with our Local Care Partnership (LCP) and third-sector organisations, including universities, we strive to meet the needs of our growing community.
Our dedicated area leads in mental health, health inequalities, nursing, and education drive our initiatives and projects, ensuring that our PCN and associated practices deliver the highest quality care to our patients.
A Primary Care Network (PCN) is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.
With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.
